Information We Collect
NFN collects the following information through online forms: name, address, email address and phone number. NFN may also collect information about whether you or a family member have Parkinson disease (PD) and if you are a healthcare or fitness professional who works with people with PD. This personally identifiable information is used for tailoring a response and customizing programs. You will also be included on our mailing list should you choose to join.
When you make a purchase or donation online, your credit card information will be collected through our secure financial transaction pages and a transaction receipt or tax acknowledgement receipt will be sent. When an anonymous donation is made, an acknowledgement will still be sent for tax purposes, but no other follow up contact will be made. When you send a donation in honor of someone else and request that an acknowledgement be sent to the honoree, we will use the honoree’s information for acknowledgement purposes only.
We may collect both “Non-Personal Information” and “Personal Information” about you. “Non-Personal Information” includes information that cannot be used to personally identify you, such as anonymous usage data, general demographic information we may collect, referring/exit pages and URLs, platform types, preferences you submit, and preferences that are generated based on the data you submit and a number of clicks. “Personal Information” includes information that can be used to personally identify you, such as your name, address, and email address.
In addition, we may also track information provided to us by your browser or by our mobile application when you view or use the Service, such as the website you came from (known as the “referring URL”), the type of browser you use, the device from which you connected to the Service, the time and date of access, your IP address, and other information that does not personally identify you. This information may include clicks, internal links, pages visited, scrolling, searches, timestamps. We share these data with WordPress, our online website service provider, and use this information for, among other things, the operation of the Service, to maintain the quality of the Service, to provide general statistics regarding use of the Service, and for other business purposes. We track this information using cookies or small text files, including a unique anonymous identifier. Cookies are sent to a user’s browser from our servers and are stored on the user’s computer hard drive. Sending a cookie to a user’s browser lets us collect Non-Personal Information about that user and keep a record of the user’s preferences when utilizing our services, both on an individual and aggregate basis. WordPress may use both persistent and session cookies; persistent cookies remain on your computer after you close your session and until you delete them, while session cookies expire when you close your browser. Persistent cookies can be removed by following your Internet browser help file directions. If you choose to disable cookies, some Service areas may not work correctly.
For information about viewing the cookies dropped on your device, visit https://wordpress.org/about/privacy/cookies/. These functional and required cookies are always used, allowing WordPress to serve this website securely.
These analytics and performance cookies are used on this website, as described above, only when you acknowledge our cookie banner. This website uses analytics and performance cookies to view site traffic, activity, and other data.
When you submit information to this website via web form, we collect the data requested in the web form to track and respond to your submissions.
How We Use & Share Information
In general, we do not sell, trade, rent or otherwise share your Personal Information with third parties without your consent. We may share your Personal Information with vendors and other third-party providers performing services for the NFN. In general, the vendors and third-party providers we use will collect, use, and disclose your information only to the extent necessary to allow them to perform the services they provide for the NFN. For example, when you provide us with personal information to complete or verify a transaction, you consent to our collecting and using such personal information for that specific purpose, including by transmitting such information to our vendors (and their service providers) performing these services for the NFN.
However, certain third-party service providers, such as payment processors, have privacy policies regarding the information we must provide them to use their services. We recommend that you read the privacy policies for these third-party service providers to understand how such providers will handle your Personal Information.
Security and How We Protect Information
NFN takes all reasonable steps to protect users’ information from loss, misuse, unauthorized access, disclosure, alteration, and destruction. NFN has in place appropriate physical, electronic, and managerial procedures to safeguard and secure such information. NFN processes personal information in a way that is compatible with and relevant for the purpose for which it was collected or authorized by the individual. However, these measures do not guarantee that your information will not be accessed, disclosed, altered, or destroyed by breach of such precautions. Using our Service, you acknowledge that you understand and agree to assume these risks.
Online forms used to collect protected health information as defined by the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule meet the security standards set forth by the same.
